Job Safety Analysis Form

Job safety analysis (JSA) is a procedure to help integrate health and safety planning into a particular job operation. The process consists of selecting a job task to analyze, breaking the job into a sequence of steps, identifying potential hazards and outcomes, ranking the risks, determining controls to mitigate the worker’s risks, and then re-ranking the risk again after the controls are in place.

Note: It is best to complete this form at the location where the task is performed.
